Every action, movement, choice made through digital interaction, every purchase, every search on any electronic device, any search engine, every internet access, every interaction mediated by a network-connected product (cellphone, Alexa, Apple Watch, cameras, electric cars, IoT, etc.), and every interaction and communication with a digital service: produces, modifies, and exchanges data.
Data is linked to a digital identity corresponding to a specific individual, and simultaneously stored in databases and physical servers spread across the Earth. These are all interconnected directly or indirectly in dematerialized structures known as the Cloud, forming dense electromagnetic networks and invisible nodes conventionally referred to as Artificial Intelligence.

Until now, the largest clouds holding data for millions of individuals, organizations, agencies, industries, institutions, are owned by private companies: IBM, Google, Facebook, Microsoft, Amazon, Apple, just to name a few. These companies are adept at extracting (value) and aggregating data from people, strategically offering free services like “Social Media,” “WhatsApp,” “Twitter,” “Zoom,” “Google Maps,” “Instagram,” “ChatGPT4,” etc.

Millions of apps are developed daily and offered for free as trials, with the only condition being installation on our phones and access to our contacts, photos, videos, audio recordings, and cameras. To ensure privacy, GDPR regulations were established, but people often don’t read the contracts for app licenses and accept the conditions dictated by the software owner with a click, authorizing them to access and sometimes take a part of our data or the data we generate.

The problem is that this naivety, combined with widespread digital illiteracy, is leading to an increase in hacker attacks. Through a single contact with someone with limited digital knowledge, hackers manage to access private and even corporate databases, potentially disrupting or damaging complex digital systems of institutions or even entire states. The increasing complexity of our world often makes us unaware of the fact that the interdependence between the physical and digital dimensions renders each of us a potentially vulnerable node in the network, constantly exposed to risks we might not even be aware of, risks that could cause immeasurable damage such as password theft or, in more severe cases, identity theft.
To counter these dangers, each state is striving to make its digital systems more secure, reliable, flexible, and resilient, alternatives to systems produced and controlled by international big private players.

In Italy, the Public System of Digital Identities (SPID) was established in 2015, a system to securely certify and track interactions between citizens and over 22,000 Italian public administrations (municipalities, regions, healthcare facilities, schools, universities, etc.). To ensure data security, protection, and preservation, the National Cybersecurity Agency (ACN) was established in 2021, and in 2022, the National Strategic Hub (PSN) was created. This cloud will gradually store all citizens’ and public administration data, including the ecosystem of health data (EDS), fed by electronic health records (FSE) of over 60 million citizens registered in the National Resident Population Registry (ANPR).

In 2023, the National Registry of Public Administration Employees (PA) was launched. This will enable tracking and certifying data produced and/or modified by each specific professional employed by the PA who provides a particular service to a registered user in ANPR.

But how does all of this relate to blockchain and the EHR?

Since the establishment of the National Health Service (SSN) in 1978, when a person encounters a health issue and interacts with one or more healthcare professionals in a specific public or accredited private healthcare facility, information and data related to that event are recorded. This is typically done on paper, known as a medical record, which can vary even within the same facility or between different healthcare facilities.

To address these issues, the Electronic Health Record (EHR) project was initiated in 2012. This digital platform aimed to contain the medical history and health records of each citizen in standardized and native digital formats. The EHR would be fueled by interoperable, easily accessible and shareable, verified, acquired, produced, and/or modified information and data from various healthcare professionals involved in a user/patient’s care, updated in all other potential healthcare processes. However, over the following decade, each region and autonomous province in Italy developed regional EHRs with little progress, and it was only during the Covid-19 pandemic that the lack of communication between them was noticed. Realizing the state of the EHR prompted renewed interest from political representatives and decision-makers. The goals of the National Recovery and Resilience Plan (PNRR) were used to redefine the EHR in the Guidelines for EHR Implementation published in the Official Gazette on July 11, 2022. The EHR must become:

The unique and exclusive access point for citizens to the SSN services.

An ecosystem of services delivered by healthcare professionals based on data.

But within what timeframe?

As of today, the EHR remains fragmented, little known to the population, and scarcely used by both citizens and healthcare professionals, simply because it isn’t useful in its current form. It’s necessary to design and create professional support tools, involving users and professionals from the initial stages of design.
Programs to develop digital skills for both citizens and healthcare professionals are necessary, preparing them for the proper and secure use of various digital products and services that are currently not even considered but need to be continually developed and updated.

Designing and implementing an EHR with a blockchain architecture is crucial. This architecture would move beyond the current logic of storing health documents and focus on data tracing. It would be necessary to redefine Essential Levels of Care (LEA) and update the index of services provided by each structure, service, and healthcare professional.

Integrating these elements would allow citizens to access a public or accredited private structure via the web, select a service, choose a procedure directly linked to the professional who will perform it. This should be connected to privacy consent, authorization to proceed, payment, and all the data recorded in the EHR after the procedure is completed. This approach could reduce bureaucracy, errors, waiting lists, and corruption associated with services provided by public administrations and accredited private organizations in the healthcare field. Additionally, it could finally enrich the electronic health record with qualitative and quantitative information useful to both citizens and healthcare professionals involved in prevention, diagnosis, care, and rehabilitation processes throughout life.

In this scenario, the characteristics of the EHR should be defined and standardized nationally, as for over 20 years, the efforts by regions in this regard have been unsuccessful and a waste of resources.

The EHR should have a unified graphical interface, be accessible to both healthy and ill citizens, and have modular functions that can be added depending on the complexity of health conditions and personal characteristics. As dozens of electronic devices equipped with sensors to continuously acquire, record, and monitor the health of elderly or frail individuals at home are produced, certified, and connected, the data feeding citizens’ EHRs will exponentially increase.

Billions of data points every day will need to be verified, interoperated with other data, and simplified to be useful for professionals in making complex and immediate decisions. However, technology alone will not be sufficient to effectively and securely assist millions of citizens over the age of 65, sick individuals, or those with vulnerabilities, a number expected to increase in Italy in the coming years.

With the PNRR, 9,600 family and community nurses (IFeC) are planned, who, along with general practitioners (MMG) and choice pediatricians (PLS), will take care of those in need of continuous and domiciliary healthcare assistance. These professionals will necessarily need to communicate, record, and trace data related to each interaction with the user in the EHR of the assisted individual, in order to measure, evaluate, and share with other healthcare professionals.

Imagine a citizen connected through their EHR to Territorial Operative Centers (COT), which in turn are connected to Community Houses (CDC). These structures, envisioned by the PNRR, will enable the SSN to offer tele-assistance, tele-monitoring, tele-consultations, and tele-medicine services.

When COTs receive data indicating an alarm, an operator active 24/7 will remotely verify the functioning of electronic devices and the person’s health status. If the alarm is confirmed, the COT should quickly inform IFeC, MMG, or PLS, who, in turn, should be able to call/prescribe/order another healthcare professional to intervene at the user’s home (nurse or socio-healthcare operator – OSS) in a timely manner. The professional should assess the health status and proceed with appropriate interventions.

This is where the concept of “accountability” comes into play, linked to the blockchain architecture in the electronic health record. Accountability refers to the principle where a subject is answerable to another subject who has recognized a specific responsibility. This principle is associated with quality, transparency, responsibility, and open data. It can apply to a company, an agency, a public administration, or a professional. The blockchain architecture, on the other hand, will be the means by which information blocks and data recorded in the EHR, associated with each interaction linked to a specific business process, securely trace the history of events, consent, authorization, and responsibility of every healthcare professional accessing the EHR, whether to view existing data or input new data.

In conclusion, there is a lot of work to be done, and the success of each project will only be possible when integrated into a shared national political vision.
This vision should focus not only on gathering votes for upcoming elections but should be driven by the opportunity to improve the living, working, and health conditions of all individuals. To stay on course for this design, which will yield results in the short, medium, long, and very long term, we must start by considering the EHR as the tool to archive the health and illness history of those born in 2020. It will be easier for all if we design health products and services centered around the most vulnerable individual: the healthy or sick newborn. However, digital transformation will become innovation when it becomes cultural transformation, and services offered to the resident population extend to all individuals temporarily residing or passing through the national territory, including students, tourists, immigrants, and war refugees.

Giovanni Stani

Blockchain and electronic health record: digital transformation will be innovation when it becomes cultural and extended to all.

[525
794,820,170,50,808,810,814,183
948,1176
post-12663","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-real-applications-en","tag-ai-eng","tag-artificial-intelligence","tag-blockchain-en","tag-cloud","tag-exchange-en","tag-facebook-en","tag-hacker-en","tag-pos-en
["GET
